- 1). Check the "Dissolution" box at the bottom of the caption that appears on the first page of the form.
- 2). Check the appropriate box that appears below the "Dissolution" box checked under Step 1. Check the "Status" box if your divorce petition did not ask the court to award property, spousal support or child custody. Check the "Reserving jurisdiction over termination of marital or domestic partnership status" if your divorce petition asked the court to reserve jurisdiction over some issue regarding the divorce, such as the division of a retirement account.
- 3). Check the box at item 1 if you are asking for a restraining order or want to change an existing restraining order. Also check either the box next to "contains personal conduct restrain orders" if the divorce judgment asks for a restraining order against the other spouse, or "modifies existing restraining orders" if you are asking the judge to change already-existing retaining orders. If you ask the court to modify a restraining order, you must attach a copy of that order and note the date when those orders currently expire.
- 4). Check the "Default or uncontested box" at item 2.
- 5). Enter the date that your spouse was served with the divorce paperwork at item 3 and check box a.
- 6). Check box a. under item 4, then check box a (1) and enter the date that your spouse was served, plus six months. For example, if your spouse was served on January 1, enter July 1 in this field. If it has been more than six months since your spouse was served, leave this field blank.
- 7). Check box f. under item 4 if your petition asked that your last name be changed back to what it was prior to your marriage, then enter that name in the provided field.
- 8). Check box g. under item 4 if you are asking the court to reserve jurisdiction over some issue in the divorce case.
- 9). Check box h. under item 4 if you are asking for child support or family support.

Check box i. under Item 4 if you and your spouse have a written settlement agreement. You must attach a copy of the agreement to this form if this box is checked. - 11
Check box k. under item 4 if you and your spouse have had children during the marriage, then enter the names of the children and their dates of birth in the applicable fields. - 12
Check the applicable boxes at items 4l, 4m, 4n and 4o. Note that the options that appear under each section are the same. Check box (1) if you have a settlement agreement. Check box (2) if you do not have a settlement agreement. - 13
Count the number of pages attached to this form and enter that number at item 5.
