- 1). Ensure that you or your organization has not already been assigned an NPI by searching your records or by searching online at www.npinumberlookup.org. Assuming you or your organization does not already have an NPI, apply through the National Plan and Provider Enumeration System (NPPES) website at https://nppes.cms.hhs.gov. Click "Apply Online for NPI" link and begin the application process.
- 2). Outsource your NPI filing request to a third-party electronic file interchange organization to obtain an NPI for you. These organizations will send you a notice with your name, address and NPI.
- 3). Apply for an NPI by filling out a paper application (Form CMS-10114) and mailing the completed application to the NPI enumerator at the following address:

Access the application online once it has been processed at https://nppes.cms.hhs.gov or call 800-465-3203 or TTY 800-692-2326. As of 2010, you also can email an inquiry about your application status to customerservice@npienumerator.com
