Assessing Unit Dose Discrepancies
Unit dose drug distribution systems that are unable to deliver the needed dose at the appropriate time may have medication error rates three to five times higher than other hospitals. Our experience and that of others suggest that missing doses commonly occur with unit dose systems. The objective of our study was to rapidly determine the magnitude of selected discrepancies in our unit dose system.
