- 1). Visit the website of Saskatchewan's Education Department. Near the bottom of the page, click on "Request for High School Transcripts (Grades 10-12)." At the very bottom of the page is a form titled "Request for High School Transcript Form."
- 2). Download and print the request form. The form cannot be filled out or submitted online.
- 3). Complete the request form by filling out the student's name, permanent mailing address and birth date. Indicate the name of the last Saskatchewan school the student attended, along with the last grade and year of attendance.
- 4). Select the transcript option that best fits your needs. The first option is for the student to receive one copy of the transcript and have a copy sent to an educational institute. The second option is for the student to receive three copies directly.
- 5). Select when you want the transcripts to be produced. The options are now, after semester results and after summer school or August results.
- 6). Determine the fee to attach to the request form. For the first option, the fee is $15 for each educational institution the transcript is sent to and the fee for the second option is $15. Additional copies mailed to student are $1 each as of 2010.
- 7). Attach a check or money order made payable to the Minister of Finance, Government of Saskatchewan.
- 8). Mail the request form and payment to the address indicated on the form. Allow two weeks for processing and delivery. The request form must be signed by the student.
