- 1). Gather all pertinent information. Names, titles, company information, witnesses name and contact information, and dates (of alleged discrimination) are all important information that the EEOC will need to process a complaint of discrimination
- 2). Complete the EEOC's online assessment. This assessment tool ask questions concerning how many employees the company has, the basis of discrimination, and the date when the discrimination occurred.
- 3). Complete the intake questionnaire and print it out. This is available once the online assessment has determined that EEOC enforces the laws that may apply to the situation being reported.
- 4). Mail in the completed intake questionnaire or take it to the nearest EEOC office. Before completing the intake questionnaire, the system will ask for a zip code and state. Once this information is entered, the system will display the mailing address for the EEOC office that has jurisdiction over the claim of discrimination. Only mail or take the intake questionnaire to the EEOC field office that is provided by the online system.
