- 1). Locate the amplifier's nine-digit serial number on the back of the unit.
- 2). Go to the QSC warranty determination page listed in resources and enter your nine-digit serial number into the search box on the page to determine if your amplifier is still under warranty.
- 3). Fill out the RMS request form located on the same web page. Fill in all of the required information on the form and click "Submit" to receive the RMS number. If you do not want to use the online form, you can contact QSC at 800-772-2834.
- 4). Print the RMS approval, tape it to the box with the broken amplifier in it and then mail it back to the address listed on the RMS approval.
