Many health conditions are managed on a daily basis from home or from an assisted living residence. In some cases, the patient manages the care based on instructions from his healthcare team and physician. In other cases, a patient actually receives scheduled visits from a home healthcare aid of some kind, such as an RN, LPN, or CNA for example.
Other possible scenarios include a hybrid of home healthcare such as assisted living. Many residents in assisted living still administer their own medications with little involvement from staff. And the newest emerging model of home healthcare includes connected health. Sometimes called "mhealth," "mobile health," or "telehealth," connected health connects the patient to her doctor and healthcare team by daily text message coaching and encouragement, disease-state progress tracking over the internet, and smart phone apps.
